Attached is the Key for the pre-post survey for RLNC and SCC along with the differences between the surveys.

1. Text box to be added on 1st page for “Unique ID” for pre/post surveys
2. Keys code “in red” be added to the database so it’s reflective when downloading data into excel (see excel spread sheet atached).
3. Numbers added to the database per survey.
